ADMISSION OF PATIENT IN INTENSIVE CARE UNIT

Purpose

To ensure that the patient undergoes admission formalities without any problems

Scope

All in-patients admitted in the Hospital

Definition

Receiving a patient to get admitted in the intensive care unit for observation, investigation, diagnosis, treatment and care

Applicable Areas

Patient admitted in intensive care unit

Responsible person

Staff Nurses, Head Nurse, Intensivist, ICU Registrar, Ward Clerk and Ward Attendants

Procedure

Following Requirements / Equipment should be ready 
 Oxygen Supply, Face Mask
 Suction Ambu Bag + Face Mask
 Pulse Oxymeter, Ventilator
 Cardiac Monitor + Electrodes
 Crash Cart
 Sphygmomanometer, Stethoscope
 IV Fluids, IV Stand, IV Tray
 Drugs – Sedation, Muscle Relaxants
 Flow Chart
 Transducer, Pressure Bag
 Thermometer
 Tape Measure
 HGT Machine with Strip
 ECG Machine
 Gloves
 Kidney Tray
 Disinfectant for Hand Hygiene

Process

 Collect information about the patient prior to admission.
 Age, Diagnosis, Respiratory Status, Cardiovascular Status, Intravenous Lines, Intra Arterial Lines, Infusions in progress any special requirements, Medical & Surgical History.
 Check all the equipment are available and functioning appropriately.
 On admission of the patient, the nurses should make a Quick Assessment of the patient’s general condition.
 Circulation, Airway, Breathing, Level of Consciousness.
 If life threatening complications exist treat immediately.
 If no immediate Life threatening complications, continue with admission procedure.
 Transfer the patient from trolley to bed, taking care of all IV lines, drainage tubes and airway.
 Connect patient to cardiac monitor.
 Remove all the valuables and hand over to the relatives, take sign and date on the admission and discharge register in the patient’s valuable column.
 Establish appropriate Oxygen Therapy – continuation of therapy already in progress of initiation of preferred method of oxygen delivery – Mechanical Ventilation, Face Mask, Nasal Cannula, T-piece.
 Connect transducers to any monitoring line – Arterial Line, Central Line, etc.
 Record Baseline observations on flow chart.
 Commence IV Infusions as ordered + medications.
 Assist Doctors in necessary procedures – Intubation / Cannulations / Catheterization, etc.
 Proper Documentation in Nurse’s Notes.
